HOUSE GUIDELINES

Good Works Recovery offers transitional living to individuals seeking recovery from alcoholism and addiction. Residents are required to be active participants in their recovery. We are here to help you achieve ongoing recovery and a fulfilled life. Also, it is our responsibility to hold you accountable to these rules and guidelines:

Violation of the following rules is grounds for immediate discharge from A Good Works home. Please note that if you are mandated by parole, probation or court, and you are discharged for any reason, that the incident will be reported immediately to the supervising officer. Also- Parole/Probation officers are granted access to the house and info on all residents at any time.

1. No drug or alcohol use. Residents must submit to urine drug screens upon request. Refusal to submit to a drug screen will be considered an admission of using. Once asked for a urine sample, you must produce it within 1 hour from the time asked; you may not leave the common area until results are finished. A relapse will result in immediate discharge.

** If management suspects you are using or drinking, a positive urine result is not required for discharge.

2. If a resident relapses, he/she will only be allowed back to a Good Works recovery home after successful completion of Good Works Transition House (2 weeks) or an inpatient treatment facility and careful consideration by management.

3. Non-residents are only allowed in the common areas of the house but not after curfew. No one is ever permitted in another person’s bedroom.

4. No stealing, fighting, or threats of physical violence. Any derogatory slurs of any kind will not be tolerated. No paraphernalia/weapons/toy weapons/or replicas are allowed on a Good Works property at any time.

5. No participation or involvement in any illegal activities will be tolerated.

6. Residents are not permitted to share the code for the door locking system.

Violation of the following rules carries various consequences, including restrictions, contract, and/or

discharge from the program, depending on severity and frequency of infractions.

7. All residents are required to attend in-house meetings, we will discuss our recovery progress as well as the house operations. In house meeting schedules are posted at each Good Works house.

8. There will be a probationary period of 30 days in which resident cannot have a vehicle or apply for overnight passes.

9. AA/NA Meetings are an important part of recovery and a requirement while living at Good Works. A minimum of 5 meetings per week are required.

10. Residents without a sponsor must obtain a sponsor within 7 days. Residents should be working the 12 steps with and communicating with sponsor.

11. All Residents are required to work or to be looking for work. Any unemployed or residents on disability must do some sort of volunteer work. Upon competition of PHP Residents must be out of the house from 9:00-5:00 Monday-Friday seeking employment if unemployed.

12. Residents are required to adhere to curfew. Exceptions are made on a case by case basis by the house manager. Sun - Thurs 11:00 PM Fri and Sat 1:00 AM

13. Lights Out is at 11:30 p.m. during the week (Sun-Thurs). Lights Out requires noise to be kept to a minimum.

14. All beds MUST BE MADE before leaving your bedroom for the day. Failure to do so will result in consequences to be determined by management.

15. Residents are required to participate in the cleaning of their house. House chores will be posted on the white board. Residents must mark the “date completed” section of the board upon completion of chore. If asked by the house manager to do a chore, resident should complete said chore within the hour and mark the white board.

16. Residents are responsible for keeping their rooms clean. A resident must have belonging stored neatly. All rooms are subject to random inspection. This includes drawers, closets, etc.

17. No overnight guest(s) will be permitted in A Good Works home.

18. Television and stereo volumes should be at a respectful level to others in the house.

19. Computers are to be used for job search, school, and other necessities. A sign out sheet will be used when necessary for computers.

20. Eating is permitted in the kitchen or dining room only.

21. No pornographic materials are permitted.

22. Residents are personally responsible for the immediate repair or replacement of property missing or damaged by their negligence. No one is to hang anything on walls without approval from management.

23. Residents are responsible for their own transportation. (Everyone is expected to help each other get to mtgs.) No personal vehicles are to be driven without providing Good Works with proof of a valid license, registration and insurance on the vehicle to be driven. Residents can only have one vehicle on the property.

24. Overnights are only to be taken with a 48 hour notice and a manager’s sign-off (must meet certain requirements.) A resident may take an overnight pass no more than twice per month. Prior to leaving the resident must leave a 10 dollar deposit for the urine to be taken upon return.

25. No smoking in houses at any time. No loitering in front of house. You must smoke or talk on cell phone, etc. in the back of the house.

26. No space heaters, humidifiers, personal refrigerators, etc. are permitted.

27. Residents are to label personal food with a marker. Any food that is not labeled is considered house food available for all residents.

28. No overnight passes are permitted to residents that owe back rent or are on behavioral contract.

29. Residents are not to begin any intimate relationships during their first 6 months living at a Good Works home.

30. Rent is to be paid on Fridays for the upcoming week. Residents must be on time with rent payments or discuss a plan for payment with management.

31. Good Works Recovery may implement other rules or regulations on an individual or the Community as it deems necessary, and reserves the right to use our interpretation of these rules.

32. It is a requirement of Good Works Recovery houses that any tenant who has been prescribed IOP, PHP, or any other type of aftercare must attend until the aftercare provider determines they have finished. In addition, anyone not involved in treatment upon enrollment to Good Works Recovery must attempt to participate in some type of outpatient counseling/ therapy. (Good Works Staff will direct you to appropriate channels for application.)

33. The last weeks rent included in the intake is only applied to a resident’s last week at the house. Residents must give a two week notice of move out.  No refunds will be given if a resident is asked to leave, relapses, or does not give the appropriate notice.

34. After a resident moves out of any Good Works home, they must collect their belongings within 7 days. After the 7 day period, their belongings will be donated to charity.

The above listed rules were explained to me and I understand them. I agree to follow these rules during my stay at Good Works.

I understand that I will be required to remain drug and alcohol free while living at Good Works and I agree to submit to a urine test at any time and that refusal will result in my immediate discharge, and agree to leave Good Works peacefully if a positive result comes up.

I am not under the influence of any non-prescribed mind or mood altering substances at this time. I understand what I am signing. I agree to waive my right to any and all eviction procedures and will leave at management’s request.

Deposit:

Security Deposit is never refunded. It is either applied to the resident's last week rent if Good Works Recovery is given a two week notice of the resident leaving, or it is held in the event a resident relapses, leaves suddenly, or is responsible for damages.

Additional Rent:

All payments are applied first to any outstanding balances and then to any current amounts due. Rent is due on a weekly basis (i.e., from Friday to Friday) and is due on the Friday immediately preceding the following rental week. There are no partial refunds available for individuals leaving before the end of the rental week and/or prior to the full term of the rental agreement.

Resident Funds:

The Resident Funds Account is an additional service that Good Works Recovery provides for our residents. All payments for the Resident Funds Account are subject to an administrative fee, which will be charged in addition to the amounts to be allocated to the individual resident's cash account. Any credits will be refunded upon the completion of the terms of the Residential Lease, subject to any necessary cleaning charges and/or outstanding rent balances.

Partial Refunds:

Any partial refunds are at the sole discretion of Good Works Recovery and will be made within 30 days electronically to the card or bank account from which they originated. In the event we process a refund, a Refund Notification will be forwarded to the email address that the Registered User (Payee) entered during registration.

ACH Debit Authorization:

I authorize my bank to debit my account as identified above to the terms stated here. This authorization shall remain in effect until the Service Provider and bank receive written notification from me of intent to terminate at such time and in such manner as to afford the Service Provider and bank reasonable opportunity to act (Minimum 30 days).

I understand that if the total amount owed to the Service Provider is increased, I authorize this plan to continue as long as the payment amount remains unchanged until the amount owed the Service Provider is paid off, or unless the plan is terminated earlier by me as above. I understand any added amounts can be applied for with a new ACH Debit Authorization Form.

All other changes such as payment amount, frequency, bank account number change, will require a new ACH Debit Payment Authorization Form to be filled out and submitted to Merchant 15 days prior to any change being implemented. I understand that this payment plan may be cancelled by the Service Provider or Merchant due to NSF (Non-sufficient Funds). I will be liable to pay an NSF fee of $25.00 (or the amount allowable by law), which may be automatically debited for each NSF.

I represent and warrant that I am authorized to execute this payment authorization for the purpose of implementing this payment plan. I indemnify and hold the Service Provider, the bank, and Merchant harmless from damage, loss or claim resulting from all authorized actions hereunder.

Acknowledgement & Acceptance of Terms:

All Internet transactions are final, and are not subject to reversal and/or cancellation. All Internet payment transactions are subject to a Convenience Fee equal to $10.00. This Convenience Fee is disclosed and included in the total payment amount prior to submission of your payment for approval. Note regarding "Save this information" and our "Express Checkout" feature: Good Works Recovery will not have access to this information. Your bank account and/or card data is encrypted prior to authorization. In exchange, the secure gateway returns a "token" to this payment system which can then be resubmitted with future payments in place of more sensitive information. End-to-End Encryption coupled with Tokenization ensures your data is secure.

By submitting your payment for authorization, you acknowledge acceptance of the terms & conditions as stated herein.
